Completion of this form is required on an annual basis for any private fire hydrant.
Backflow Device Inspection Report
Inspection is required annually. The inspection is required to be performed by a State licensed Sprinkler Contractor. A record of inspection is required to be forwarded to the Division of Codes and Standards. A record is to remain on premises with the occupant for review by fire inspectors.

Written in 1995 and is a standard for approved central monitoring stations for required fire alarm systems.
MFD Codes and Standards Test and Inspection Checklist
This form is provided to persons responsible for the maintenance on features of fire protection in buildings or structures. It is not all-inclusive but a general breakdown of what is required to be checked, how often and in some cases limits who can inspect these items. Features of fire protection when installed in a building must be properly maintained.
Burn Permit
Issued to allow controlled burnings under strict guidelines, the most common are noted on the permit.
Fire Watch Procedures and Agreement
If sprinkler systems or the monitoring of sprinkler systems are out of service for an extended period of time, a person designated with the sole responsibility of fire watch duties shall be required. A written agreement is required to be forwarded to the Fire Department.
