1. Employee should immediately report an on-the-job injury to supervisor.

2. Employee and Supervisor will report the injury to the 24/7 Workplace Triage Hotline at 1-855-660-5200.

3. Employees seeking medical treatment will be evaluated and submit to drug screen.

- Report to City Occupational Center (Premise Health, 310-B Hull Street) during normal business hours (Mon-Fri 7a-5p).

- If the Occupational Center is not available, or for after hours, employee will report to Jackson Hospital ER.

- Upon ER visit, employee must follow-up with Premise Health (334) 350-3050.

4. Plan of care will be assigned by the Triage Nurse.

Before an employee is authorized to drive a City vehicle, they must submit a driving permit application to the Safety Office.

The application must be accompanied by a current Motor Vehicle Record (MVR) which can be obtained from the Alabama Law Enforcement Agency (ALEA) office located at 301 South Ripley St.

The application contains a health questionnaire, a Safety Briefing, and a Backing Policy. The Backing Policy only needs to be completed if the applicant has a CDL License.
